The Counselor assists the Family Therapist in the development and implementation of the Family program for Addition patients, who are also HIV positive or have AIDS and their families; conducting information sessions and counseling patients.  The Counselor is responsible for getting the Family Resource Notebooks compiled and distributed, supervising the Peer Educators, and assisting the Outreach Coordinator in promoting the Family Program inside the Hospital and the community.

QUALIFICATIONS:

Bachelors degree, minimum three years experience, and CADC, required.  Experience working in or with a hospital-based treatment program.

The Case Manager (CM) assumes responsibility for assessing and directing the clinical management of patients in specific case groups for an episode of care.  The CM is responsible for developing and meeting desired patient care outcomes for his/her caseload based on assessment of patient care needs and on established clinical care patterns, within an appropriate length of stay and appropriate use of resources.  The Case Manager, responding to complex patient care needs from admission to discharge, works productively and cooperatively in collaboration with the physician, clinical staff nurses and managers as well as other health care professionals.

QUALIFICATIONS:

Bachelor’s degree in nursing or health related field preferred.  Registered Nurse with current license in Illinois.  Minimum of 2 years experience in Medical, Surgical or Psychiatry and/or Critical Care.  At least one to two (1-2) progressive experience in utilization management, discharge planning or case management required.  Strong communication skills, utilizes problem-solving process, acts as team leader, demonstrate good work ethics, demonstrates good nursing assessment skills, maintain confidentiality, demonstrates flexibility, and analytical skills.  Has working knowledge of intensity of services and severity of illness criteria.  Demonstrates working knowledge of prayer requirements.
